Completing the mandatory COVID-19 screening prior to check-in
Within the last 14 days, I have not travelled outside of Canada, including the United States
Within the last 14 days, I have not experienced cold, flu or COVID-19-like symptoms, even mild ones (symptoms include: fever, chills, cough, shortness of breath, sore throat and painful swallowing, stuffy or runny nose, loss of sense of smell, headache, muscle aches, fatigue or loss of appetite)
Within the last 14 days, I have not had close contact with or provided care for an individual who travelled outside Canada, or experienced cold, flu, or COVID-19-like symptoms as listed above:
Rescheduling your visit if you are experiencing, or in close contact with anyone experiencing, cold/flu symptoms and/or under self-isolation orders
Wearing a cloth/disposable face mask to your appointment
Arriving alone or limiting companions to one parent/guardian/caregiver
